Caring for the Whole You: Your Health’s Behavioral Health Program

At Your Health, we believe health is about more than managing medical conditions. How you feel emotionally, mentally, socially, and spiritually has a real impact on your physical health and your quality of life. That’s why our Behavioral Health Program is built directly into the care we provide every day.

This program exists to support the whole person to help patients feel more stable, more supported, and more confident in their care.

Why Behavioral Health Is So Important

Emotional stress, anxiety, depression, loneliness, grief, and major life changes often show up as physical symptoms. When these concerns are not addressed, patients may require more frequent doctor visits, emergency care, or hospitalization.

By identifying and supporting behavioral health needs early, we can help reduce unnecessary medical visits while improving overall well-being. Patients feel better, stay more stable, and are better supported through every stage of care.

Care That’s Fully Integrated

Behavioral health at Your Health is not a separate service or referral you have to navigate on your own. It is part of your existing care team. Your medical providers, behavioral health specialists, and support staff work together, sharing information and coordinating care so nothing falls through the cracks.

This integrated approach allows us to tailor support to each patient’s specific needs, whether that means emotional check-ins, counseling, medication support, or help connecting to community resources. Care can take place in the home, in a facility, or through telehealth—whatever works best for you.

Support From a Team That Knows You

Our Behavioral Health Program includes a dedicated team working together to support your well-being:

  • Social Services Specialists
Social Services Specialists are embedded in every care team and often serve as the first point of behavioral health support. They help identify emotional, social, and spiritual factors that may be affecting your health, such as stress, isolation, caregiver burden, or major life changes. They also help coordinate services and connect patients with the right resources and next steps.

  • Behavioral Health Counselors
Licensed counselors provide therapy and emotional support, most often through telehealth. Counseling can help patients manage anxiety, depression, grief, chronic illness, and everyday stress. Counselors work closely with the care team to ensure continuity and a personalized approach.

  • Psychiatric Nurse Practitioners
Psychiatric Nurse Practitioners support patients who need medication management as part of their behavioral health care. They focus on finding the right medication balance, monitoring symptoms, and adjusting treatment to promote long-term stability while avoiding unnecessary medication use.

  • Psychiatric Pharmacists
Psychiatric Pharmacists add an extra layer of medication safety. They regularly review psychiatric medications to check for interactions, side effects, and opportunities to simplify treatment plans. This helps ensure medications are effective, safe, and appropriate over time.

What Patients Can Expect

Patients enrolled in the Behavioral Health Program often report feeling more supported, more heard, and more in control of their care. Addressing emotional and mental health alongside medical needs can lead to fewer crises, fewer emergency visits, and a better day-to-day quality of life.

Most importantly, patients are never expected to manage behavioral health concerns alone. Support is built into the care you already receive.

A Core Part of How We Care

The Behavioral Health Program is not an add-on; it is a core part of how Your Health delivers care. By treating the whole person and addressing the factors that truly drive health and wellness, we are able to provide care that is more compassionate, more effective, and more personal.

If you or a loved one could benefit from behavioral health support, talk with your care team to learn more about what’s available through Your Health.
